Job summary

Circadia Health seeks a on-site Nurse Practitioner in Marlborough, MA to round on residents, assess needs, and coach staff in using Circadia tools. You’ll blend direct patient care with real‑time predictive health signals, training staff and integrating tech into daily workflows.

The role focuses on reducing readmissions through hands‑on care, live demos, and collaborative problem solving with clinical ops and product teams. Travel within a 30–40 mile radius is expected.

Qualifications

• MA NP license active and in good standing.
• ANCC (or equivalent) certification in Adult/Family/Geriatric/Acute.
• 2+ years NP experience with complex adult populations and independence.
• Valid MA driver’s license; comfortable traveling locally.

Responsibilities

• Direct patient care: initial and follow-up assessments; modify plans of care; coordinate with facility providers.
• Tech → bedside integration: train staff and standardize Circadia use in workflows.
• Data into action: interpret trends, prioritize high‑risk patients, drive timely interventions.
• Close the loop: complete documentation the same day.
• High‑volume rounding: 25+ visits/day across SNFs when appropriate.
• Quality & product feedback: surface field insights to improve workflows.
• Mentor & collaborate: support NP onboarding and partner with ops/product teams.
• Regional flexibility: cover sites within 30–40 mile radius; mileage lodging per policy.

The Role

Transform bedside care with AI. As Circadia’s on‑site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech‑to‑bedside integration through hands‑on training, live demos, and shadowed assessments.

In our SNF partner facilities, you'll round on new admits, facility‑flagged patients, and Circadia alerts. In the home health setting, you'll bring that same proactive model directly into patients' homes, extending high‑quality care beyond the walls of a facility.

Throughout, you'll blend direct patient care with tech‑to‑bedside integration through hands‑on training, live demos, and shadowed assessments.

What You’ll Do

• Direct patient care: Conduct initial and follow‑up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)

• Visits: Target 25 patient visits/day
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in MA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

Nice-to-Haves

• Tech‑savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

$3,000 - $5,200 a month

Compensation & Perks
