Job summary

Circadia Health is seeking an on-site Nurse Practitioner in Marlborough, MA to deliver direct patient care and lead tech-to-bedside integration. You will round in SNFs and home health settings, training staff and guiding intervention decisions based on real-time AI insights.

We value collaboration with clinical ops and product teams, strong MA licensure, and the ability to travel within a 30–40 mile radius. English fluency and startup adaptability are essential.

Qualifications

• Licensed NP in MA (ANCC or equivalent).
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations.
• Valid MA driver’s license; comfortable traveling locally.
• English required.
• Thrives in a dynamic startup environment.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Train and coach staff at the point of care; standardize Circadia into clinical workflows.
• Interpret trend-based insights; prioritize high-risk patients; drive timely interventions to prevent decline.
• Complete documentation/charts within the same day.
• High-volume rounding across SNFs (typ. 25+ visits/day); focus on new admits and Circadia alerts.
• Surface field insights to improve workflows and tech integration in clinical settings.
• Mentor NP onboarding; collaborate with clinical ops/product teams.

The Role

Transform bedside care with AI. As Circadia’s on‑site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech‑to‑bedside integration through hands‑on training, live demos, and shadowed assessments.

In our SNF partner facilities, you'll round on new admits, facility‑flagged patients, and Circadia alerts. In the home health setting, you'll bring that same proactive model directly into patients' homes, extending high‑quality care beyond the walls of a facility.

Throughout, you'll blend direct patient care with tech‑to‑bedside integration through hands‑on training, live demos, and shadowed assessments.

What You’ll Do

• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend‑based insights, prioritize high‑risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day
• High‑volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility‑flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)

• Visits: Target 25 patient visits/day
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Must-Have Qualifications

• Certified and licensed Nurse Practitioner in MA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid MA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required

Nice-to-Haves

• Tech‑savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

$3,000 - $5,200 a month

Compensation & Perks
