Job summary

Optum's Senior Community Care program is seeking a Per diem Advanced Practice Clinician (NP or PA) to deliver comprehensive care to high-risk patients in Yakima or Tri-Cities, WA. The role offers a $5,000 sign-on bonus with no on-call, no weekends, and no holidays, ensuring a predictable schedule and strong work-life balance.

Responsibilities include assessments, medication reviews, care planning, and coordination with interdisciplinary teams to improve outcomes for seniors in skilled nursing

Qualifications

EMR proficiency and technology comfort.
Active certifications for NP or PA required.
Unrestricted state license or ability to obtain one.
Driver's license and reliable transportation.
Ability to obtain collaborative practice agreement where required.

Responsibilities

Perform comprehensive patient assessments and physical exams.
Review medical histories, medications, and care plans.
Support patients during short-term rehabilitation stays.
Document diagnoses and clinical findings accurately.
Collaborate with physicians, nurses, social workers and care teams.
Conduct advance care planning and serious illness conversations.
Educate patients and caregivers on disease management and treatment plans.
Coordinate care for patients with complex medical, behavioral, and social needs.
