Job summary

Amedisys, part of the Optum family, is hiring a Per diem Medical Social Worker to provide psychosocial support, assessment and care coordination in the home. You will address emotional, social and environmental factors and collaborate with the interdisciplinary team to connect patients with community resources.

The role includes education, care planning, discharge coordination, and identifying risks to patient health.

Qualifications

• Master's and/or Doctorate degree from a CSWE-accredited school of social work.
• Active licensure in Medical Social Work (LMSW) or Clinical Social Work (LCSW).
• Driver's License and access to reliable transportation.
• Current CPR certification.
• Meets state and federal requirements for this position.

Responsibilities

• Assists patients and families with health-related financial, social and emotional concerns and coping skills.
• Provides social work services in accordance with the plan of care.
• Communicates changes in clinical status to the health care team and office staff promptly.
• Educates patients and families on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and implements interventions to reduce risks.
