Qualifikationen

• MSW or Doctorate in Social Work from CSWE-accredited school.
• Active LMSW or LCSW licensure.
• Driver's license and reliable transportation.
• CPR certification.
• At least one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.

Aufgaben

• Provide psychosocial support, assessment, and care coordination in the home.
• Collaborate with interdisciplinary team to connect patients with community resources.
• Educate patients and families on social work interventions and treatment plans.
• Support care transitions and coordinate transfer and discharge.
