Job summary

LPN/LVN Post Acute Care (Per diem)

Location: Abilene, KA

Shifts (days, Evenings and Nights (8-hour flexible schedules-per diem shifts)

Pay (Negotiable)

Key Responsibilities

Direct Patient Care

· Monitor vitals: Check temperature, blood pressure, heart rate, and oxygen levels.

· Give medicine: Administer pills, IV drips, and other treatments safely.

· Manage wounds: Clean injuries, change bandages, and track healing.

Assessment and Planning

· Track symptoms: Observe changes in health and report issues immediately.

· Create care plans: Build or update steps for patient recovery with the medical team.

· Run tests: Collect blood or fluid samples and help review results.

Communication and Education

· Talk with doctors: Share updates and discuss changes in a patient's health.

· Teach families: Explain how to care for an illness or injury at home.

· Support patients: Offer comfort and answer questions during stressful times.
