Job summary

Louisiana HomeCare, part of the LHC Group and UnitedHealth Group family, is seeking a Per Diem Licensed Practical Nurse or Licensed Vocational Nurse to provide high-quality in-home patient care.

Responsibilities include adhering to the plan of care, educating families, monitoring patient status, and collaborating with the care team. Requirements include active LA licensure, a valid driver’s license, and CPR certification or ability to obtain within 90 days.

Qualifications

• Current and unrestricted LPN or LVN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Home care experience.
• Ability to work flexible hours.
• Proven ability to work independently.
• Proven excellent communication, writing, and organizational skills.

Responsibilities

• Provides services within the scope of practice as defined by state laws governing nursing and in coordination with the health care team.
• Instructs the patient’s family in care and environment maintenance.
• Observes, records, and reports patient response and condition changes to the supervising nurse/physician.
• Works effectively in diverse home settings and with varied patient demographics.
