Job summary

Millennium Physician Group is seeking Licensed Practical Nurses to join our Home Health Team serving Sarasota County in a Per Diem (PRN) role with flexible hours. You will provide skilled nursing care under direct RN supervision, including med administration, wound care, and vital sign monitoring, and collaborate with physicians and therapists to ensure optimal patient outcomes.

The position requires a Florida LPN license and at least 6 months of clinical experience in a hospital or skilled

Qualifications

• Current and valid LPN license in the state of Florida.
• Minimum of 6 months of clinical experience as an LPN in a hospital or skilled nursing facility.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide high-quality nursing care including meds, wound care and vital signs.
• Conduct clinical assessments and develop care plans.
• Administer prescribed medications and educate patients on regimens.
• Implement treatments such as IV therapy and catheter care.
• Maintain documentation of care and interventions.
• Collaborate with physicians and therapists to achieve patient goals.
• Adhere to infection control guidelines and ensure patient safety.
