Job summary

BAYADA Home Health Care is seeking a compassionate Licensed Practical Nurse (LPN) for a Per Diem role serving adult clients in Annandale, VA and surrounding areas. Medicare Home Health experience is required along with at least one year of LPN clinical experience.

You will be part of a multidisciplinary team delivering skilled nursing and rehabilitative care at patients' homes, with travel across designated territories and the use of EMR on a tablet.

Qualifications

• Active VA LPN license.
• Minimum 1 year LPN experience.
• Graduate from accredited nursing program.
• Home care experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments and procedures.
• Monitor clients' conditions and report changes to supervisor.
• Follow up with and document doctors' orders.
• Perform client assessments as necessary.
• Coordinate case management and services.
• Document observations and interventions in EMR on a touchpad tablet.
