Job summary

Apple Rehab West Haven is seeking a compassionate LPN Charge Nurse for per diem shifts, supporting our 90-bed community. You will plan, coordinate, and oversee nursing services for residents within a family‑oriented environment where long-standing staff deliver warm, personalized care.

Starting pay ranges from $30 to $37 per hour based on licensure and experience. Shifts include 8am–12pm and 4pm–8pm, with per diem staff expected to fulfill two weekend shifts monthly.

Qualifications

• Valid state nursing license in good standing (or in the process of becoming a registered nurse).
• In-depth knowledge of nursing practices, medical procedures, and long-term care regulations.
• Strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks.
• Collaborative team player with previous experience in long-term care preferred.

Responsibilities

• Plans and provides professional care for residents in line with quality care standards and facility policies.
• Maintains professional interactions with residents, physicians, families, peers and staff.
• Documents crucial resident care information accurately and comprehensively.
• Administers prescribed medications and maintains records per facility policies.
