Job summary

Chapters Health is seeking a Per Diem LPN in Florida under RN supervision to provide nursing interventions and teach skills to maximize patient comfort and quality of life for patients and families.

The role requires Florida LPN licensure, IV certification, BLS, and a flexible schedule to meet patient needs. Empia experience in infusion therapy is a plus; driving to patient visits is expected.

Qualifications

• High school diploma or GED or equivalent
• Current LPN licensure in the state of employment
• IV certification required at time of hire
• Active BLS for healthcare professionals
• For Hope Employees: BLS by 2025-01-01
• For Capital Caring employees: BLS by 2024-11-01
• Mobile Driver - valid driver’s license and automobile insurance
• Reliable transportation to meet visit schedule
• Infusion Therapy course and in-house infusion class preferred
• Ability to communicate in English
• Flexible schedule to meet patients’ needs
• Ability to use equipment with visual/auditory mechanisms
• Ability to manage emotional stress of caring for terminally ill
• Ability to perform essential functions and physical requirements
• Can work within state regulations

Responsibilities

• Under RN supervision, provides nursing tasks to patients and families per the plan of care
• Demonstrates clinical skills including medication administration and symptom management
• Observes/monitors patients for changes and reports to RN
• Reports adverse reactions to treatments; coordinates with physicians/APPs
• Performs venipuncture for blood specimens
• CVL site observation and IV administration per procedure
• Documents in the medical record per policy
• May be required to report to work during emergencies to ensure continuity
• Performs other duties as assigned
• Some locations may require cross training to Hospice House roles

Role:

Under RN supervision, the Per Diem - LPN is responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.

Qualifications:

• A high school diploma or G.E.D. or an equivalent combination of education and experience
• Current LPN licensure in the state of employment
• IV certification required at time of hire beginning November 10, 2024. Employees in this job prior to November 10, 2024 will be required to obtain certification by June 10, 2025.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• For current Hope Employees: BLS should be obtained by January 1, 2025
• For current Capital Caring employees: BLS should be obtained by November 1, 2024

• Reliable transportation to meet visit schedule
• If working in an inpatient unit certificate of completion of 30 hour Infusion Therapy Course and in-house 8-hour Infusion Class preferred

Job Responsibilities:

• Under the direction of an RN, provides nursing tasks to patients and families such as continuous care, dressing changes, catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).
• Demonstrates clinical skills regarding application of Company policies and procedures such as: medication administration, symptom management, and patient/family relationships.
• Observes/monitors patients for alterations in body systems, pain and symptoms and reports to RN.
• Observes and reports signs of adverse reactions and responses to treatments and reports to RN.Reports findings to physicians, APRNs and physician’s assistants following collaboration with RN.
• Performs venipuncture for blood specimens.
• Performs Central Venous Line (CVL) site observation/inspection; venous access device dressing, cap change and flushing venous access device; IV medication and fluid administration, IV pushes (per procedure) and venipuncture. May not make any changes to infusion pump including, but not limited to, rate changes and IV bag changes.
• Documents per policy and procedure in medical record.
• During times of emergencies (i.e. Hurricanes, etc.), the LPN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency
• Performs other duties as assigned.
• Some locations may require:Cross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing.
