Job summary

OneWell Health Care is seeking a Per Diem Housing Transition Assistant in Downingtown, PA. You will support individuals with IDD in securing and maintaining housing, including applications, tours, and move-in logistics.

This field-based role involves coordination with a Housing Coordinator and team leads to promote independent living skills. Candidates should have a high school diploma or GED, a valid driver’s license, and strong documentation abilities.

Qualifications

• High School Diploma or GED required.
• Valid driver's license and reliable transportation required.
• English language required; additional languages are a plus.
• Experience in IDD, behavioral health, housing services, or human services preferred.

Responsibilities

• Help individuals search for housing options and complete applications.
• Coordinate housing tours and move-in logistics, including utilities.
• Support tenancy sustenance and independent living skills.
• Maintain timely documentation in C2S and other systems.
• Communicate with stakeholders and support audit readiness.
