Duties/Responsibilities
  • You will be responsible for all aspects of your assigned caseload, including meeting with clients regularly and ensuring all clinical files and reports are complete and up to date.
  • You will engage individuals in a client-centered approach and provide services and referrals to address housing needs, track trends in placements, and orient clients with the housing group.
  • Secure entitlements, maintain correspondence related to housing interviews and vacancies, and assist clients in coordinating moves with landlords and agencies.
  • Serve as a liaison with DHS and other providers to facilitate housing processes.
  • Provide housing retention services after placement and participate in team meetings and case conferences.
  • Provide emergency first aid/CPR assistance when needed.
  • Perform related duties as assigned.
Qualifications
  • High School Diploma/GED required; Bachelor's Degree preferred.
  • Experience working with individuals experiencing homelessness, securing entitlements, and knowledge of mental illness and substance abuse is helpful.
  • Strong writing, communication, and group facilitation skills.
  • Bilingual abilities are a plus.
  • Proficiency in MS Office, including Word and Excel.
  • CPR training certification or willingness to complete certification.
