Job summary

Qualifications

• Master's Degree in Social Work required.
• LSW or LCSW required.
• Minimum one (1) year hospital social worker experience with additional behavioral health experience a plus.
• Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).

Responsibilities

• Provide emotional support and crisis intervention to Pediatrics, Adolescents, Adults, Older Adults and families during hospitalization.
• Assist in case management, develop and reevaluate Treatment Plans and coordinate discharge planning.
• Complete necessary documentation and maintain clinical records for each client during length of stay.
