Job summary

Huntington Hospital is seeking a Discharge Planner Per Diem in Pasadena to support discharge planning under Case Management. You will coordinate patient transitions from inpatient care and act as a liaison to external case managers, payors, and community resources, with dayshift hours.

Required are a social work degree or LVN with related experience and strong communication, coordination, and computer skills.

Qualifications

• Bachelor’s Degree in Social Work or related field; LVN acceptable.
• BSW with 1 year healthcare experience; new grads with healthcare field placement preferred.
• Bachelor’s in social field: 3+ years in case management/utilization management preferred.
• LVN: 3+ years in case management/utilization management preferred.

Responsibilities

• Discharge planning interventions under the direction of Case Managers and Social Workers.
• Coordinate patient movement out of the inpatient hospital setting.
• Liaise with external case managers, payors, and community resources.
• Serve as internal resource to PFS around discharge planning; liaison to external entities.
