Job summary

Longwood Hospice - North Shore Area is seeking a compassionate Registered Nurse (RN) to join our hospice team providing high-quality, patient-centered care during end-of-life. This per-diem role supports visits, admissions, and plan updates, with autonomy and collaborative team care.

The ideal candidate has an active RN license, at least 2 years of nursing experience, strong communication skills, and comfort with EMR systems. Travel to patient homes is required.

Qualifications

• Current Registered Nurse (RN) license in good standing.
• Minimum 2 years of nursing experience.
• Strong clinical, communication, and interpersonal skills.
• Ability to work independently with minimal supervision.
• Basic computer proficiency (EMR systems).
• Must pass background check and pre-employment requirements.

Responsibilities

• Provide direct nursing care through routine visits, admissions, and recertifications.
• Perform comprehensive patient and family assessments and develop individualized care plans.
• Conduct pain and symptom management assessments at every visit.
• Communicate changes in patient condition to the RN Case Manager and physician as needed.
• Maintain accurate, timely documentation in accordance with hospice policies.
• Participate in interdisciplinary team (IDT) meetings.
• Support patients and families during end-of-life care, including presence at time of death when possible.
• Supervise LPNs and Home Health Aides as needed.
• Ensure compliance with all regulatory, HIPAA, and infection control standards.
• Participate in ongoing training, quality improvement, and professional development.
