Job summary

Chapters Health System in Georgia is seeking a Per Diem Registered Nurse to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Group, and provide palliative and supportive care in the Hospice House.

The RN will document care, use SBAR for communications, and support physicians and staff. A current RN license in Georgia, BLS certification, and 1 year nursing experience (hospice or hospital preferred) are required.

Qualifications

• Active RN license in Georgia.
• BLS for healthcare professionals (AHA/Red Cross).
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.

Responsibilities

• Assess patient and family needs using the nursing process.
• Coordinate Plan of Care with Interdisciplinary Group (IDG).
• Provide palliative and supportive care to patient/family.
• Complete admissions and documentation per policy.
• Communicate with physicians and staff using SBAR.
• Support bereavement and discharge planning as needed.
