Job summary

Continuum Hospice is seeking a Per Diem Compassionate Resource Nurse (RN) to provide patient care in and around the Concord area. The Resource Nurse collaborates with the intake team, referring physicians, and Hospice Medical Director to promote the hospice philosophy of care.

The role covers pre-assessments, admissions, and in-home visits, with responsibilities including nursing care, family education, and ensuring certifications.

Qualifications

• Must hold a current unencumbered RN license.
• Bachelor's degree in nursing or 1 year nursing experience within last 3 years.
• Hospice/home health experience preferred but not required.
• Proficient in computer typing and EMR.
• CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Reviews referral information and collaborates with Clinical Director on eligibility.
• Provides pre-assessment, admission services in home or facility as needed.
• Coordinates with Intake and referring physician to secure necessary certifications.
• Applies nursing care and education to patients and families; supports plan of care.
• Participates in on-call duties as assigned.
