Job summary

Pathways Healthcare is a Massachusetts-based home health provider seeking a Per Diem Registered Nurse to deliver high-quality in-home nursing services in the Westwood, Quincy, Brockton, Weymouth, and Dover areas. The role involves pediatric and geriatric care planning, patient assessments, medication administration, and collaboration with medical teams.

Applicants must hold an active MA RN license, with 2+ years in geriatrics preferred, and a valid driver’s license.

Qualifications

• Active nursing license in the state of work (MA).
• 2+ years nursing experience, geriatrics preferred.
• Nursing degree from an accredited institution.
• Excellent bedside manners and strong communication.
• Knowledge of healthcare laws and regs.
• Current BLS CPR certification.
• Valid driver’s license and own reliable vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create Plans of Care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of the care plan.

• Possess a valid driver’s license and own a reliable, insured vehicle.
