Job summary

Heart of Hope Asian American Hospice Care in San Jose, California is seeking a Medical Social Worker (MSW) to join our nonprofit team providing end‑of‑life care to underserved communities. You will join the hospice interdisciplinary team to support patient services and counseling for patients and families, including bereavement support.

The ideal candidate has an MSW from a CSWE‑accredited program; LCSW is preferred but not required.

Qualifications

• MSW degree accredited by CSWE
• Licensed Clinical Social Worker (LCSW) preferred but not required
• Minimum one year healthcare/hospice experience
• Strong verbal and written communication skills

Responsibilities

• Support patient service activities as part of the hospice interdisciplinary team
• Provide social service counseling to maximize patient and family coping
• Deliver bereavement counseling to family members
