The Elizabeth Hospice is seeking a Per Diem LVN to provide professional nursing care under RN supervision for patients across the lifespan in homes or facilities. The role requires travel to patient sites in San Diego and Riverside counties with evenings/nights possible.

Current California LVN license and CPR are required; pediatric experience and bi-lingual Spanish are a plus. We offer competitive hourly pay, shift differentials for late hours, and a comprehensive benefits package for eligible

Competenze

• CA LVN license current and in good standing.
• Minimum 12 months LVN experience; home health or hospice preferred.
• CPR certification current and maintained.
• Bilingual Spanish desired.

Mansioni

• Assist RN in assessing patients and planning care.
• Evaluate patient responses toward expected outcomes.
• Perform licensed nursing skills per policy and procedure.
• Follow the RN plan of care and document care provided.
• Teach patients and families under RN supervision, documenting progress.
• Document all care and maintain timely charting.
• Enter/retrieve data in the electronic charting system.
• Orient/train assigned personnel and perform phlebotomy as needed.
• Support audits and office duties; comply with E-way standards.
