Job summary

MATRIX HOME CARE is hiring licensed Physical Therapists for per diem home health visits in Pinellas County, FL. The PT evaluates patients, develops individualized plans, and delivers skilled therapy in patients’ homes to improve mobility and independence.

Ideal candidates are dependable, compassionate, and able to work independently in the field, with strong documentation and communication skills and compliance with regulations.

Qualifications

• Active Florida Physical Therapist license in good standing.
• Home health experience preferred.
• Experience with evaluations, treatment planning, gait and balance training, and fall prevention.

Responsibilities

• Complete physical therapy evaluations and reassessments in the home setting.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide skilled therapy services to improve mobility, balance, strength, endurance, safety, and independence.
• Educate patients and caregivers on home exercise programs and safety precautions.
• Monitor progress and update the plan of care as needed.
• Communicate with physicians and care team regarding patient status and changes.
• Complete accurate and timely documentation.
• Follow state/federal regulations and home health standards.
