Job summary

@Home Therapy in Boca Raton, FL is seeking a compassionate Occupational Therapist to provide one-on-one home health services. You will evaluate patients in their homes, design tailored treatment plans, and support independence in activities of daily living.

Ideal candidates have FL OT licensure and strong clinical reasoning. Per diem scheduling offers flexibility, with administrative support and a user-friendly documentation system to streamline referrals and care coordination.

Qualifications

Current FL State Occupational Therapist license.
Strong clinical reasoning and communication skills.
Ability to work independently in a community-based setting.
Experience with adult or geriatric patients is preferred but not required.
Home health experience is welcome but not necessary.

Responsibilities

Perform occupational therapy evaluations in patients' homes.
Design individualized treatment plans based on each patient's functional goals.
Address limitations affecting self-care, mobility, and everyday activities.
Recommend adaptive equipment, home modifications, and safety strategies that support independent living.
Educate patients, family members, and caregivers on techniques that promote long-term success.
Monitor patient progress and update treatment plans as needed.
Complete timely electronic documentation in accordance with Medicare and agency guidelines.
