Job summary

Klarus Home Health is seeking a PRN/LVN to deliver patient-centered nursing care in home settings across the Dallas, TX region and surrounding areas. The LPN/LVN will work under a registered nurse to coordinate and implement care plans, ensuring optimal outcomes for diverse patients.

The role requires an active LVN license, two years of clinical LVN experience, and the ability to travel locally. HCHB experience is highly preferred, with emphasis on safety and high-quality documentation.

Qualifications

• Graduate of an accredited School of Vocational/Practical Nursing.
• Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license via NLC.
• Two years’ LVN experience in a clinical setting; home health preferred.
• HCHB experience preferred.
• Proficiency in nursing skills and good interpersonal skills.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Provide nursing care to patients in their homes under RN supervision.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Assist in developing and implementing patient care plans.
• Monitor patient health and report changes to the supervising RN.
• Administer medications and treatments as prescribed.
• Educate patients and families on health care needs and options.
• Maintain accurate and timely documentation of patient care and progress.
