Job summary

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current New Hampshire APRN or PA-C licensure (or eligibility).
• DEA certification (or ability to obtain).
• Minimum of 2 years of experience caring for geriatric patients.
• Strong clinical assessment and decision-making skills.
• Ability to work independently in a nursing home environment while collaborating with an interdisciplinary team.

Responsibilities

• Provide comprehensive medical care to geriatric patients in skilled nursing, rehabilitation, and long-term care settings.
• Assess, diagnose, and manage acute and chronic medical conditions.
• Perform routine follow-up visits, discharges, and transitional care management.
• Collaborate with physicians, nursing staff, rehabilitation services, social workers, and family members.
• Document care accurately and timely in the electronic medical record.
• Participate in quality improvement and patient-centered care initiatives.
• Provide outpatient Senior Health clinic coverage as needed for acute patient care needs.
