Job summary

Elliot Health System's Senior Health Practice is seeking an experienced Nurse Practitioner (APRN) or Physician Associate (PA-C) to join our geriatric care team on a per diem basis, providing care in skilled nursing facilities across Manchester and Bedford, New Hampshire.

The role emphasizes autonomous practice within an interdisciplinary team, focus on geriatric medicine, and opportunities to support our outpatient Senior Health clinic for acute needs, with a flexible schedule and no patient

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Associate program.
• Current NH APRN or PA-C licensure (or eligibility).
• DEA certification (or ability to obtain).
• Minimum of 2 years of experience caring for geriatric patients.
• Strong clinical assessment and decision-making skills.
• Ability to work independently in a nursing home environment while collaborating with an interdisciplinary team.
• Epic or similar EMR experience preferred.

Responsibilities

• Provide comprehensive medical care to geriatric patients in skilled nursing and long-term care settings.
• Assess, diagnose, and manage acute and chronic conditions.
• Perform routine follow-up visits, discharges, and transitional care management.
• Collaborate with physicians, nursing staff, rehabilitation services, social workers, and families.
• Document care accurately in the EMR.
• Participate in quality improvement and patient-centered initiatives.
• Provide outpatient Senior Health clinic coverage as needed for acute care.
