Maya Home Care in New Jersey is seeking per diem registered nurses to see patients in person across the state. This part-time role involves working in patients’ homes with flexibility for mornings Monday through Friday.

You’ll assess, implement and adjust plans of care, coordinate with DON or Administrator, and guide home health aides to ensure quality care. NJ RN license and current driver's license required; Spanish speaking a plus.

Qualifikationen

• Valid & current New Jersey Registered Nurse License and Registration.
• Current Driver’s License; Means to drive; Ability to drive up to 80 miles if needed.
• Licensed home care services (LHCSA) or Geriatric experience preferred.
• Minimum of 1-2 years clinical nursing experience.
• Spanish Speaking a plus; Road warrior.

Aufgaben

• Assess patient at start of care to develop a plan of care.
• Initiate Plan-of-Care and make changes as patient needs evolve.
• Evaluate patients in their homes.
• Instruct and advise patients and/or families and home health aides in patient care.
• Reassess patients every two months and supervise home health aide performance.
