Qualifications

• Bachelor's Degree in Social Work or related field, or LVN with related experience.
• For BSW: 1 year experience in healthcare; field placement acceptable for new grads.
• Strong communication, coordination and problem-solving with emphasis on customer service; 40 wpm typing; proficient in computer systems.

Responsibilities

• Coordinate discharge planning activities and support patient transitions out of inpatient care.
• Serve as liaison to internal staff, external case managers, payors and community resources.
• Adapt to flexible day/hour scheduling and manage multiple priorities within complex hospital processes.
