Job summary

Saint Francis Hospital and Medical Center, a member of Trinity Health Of New England, is seeking a Crisis Clinician to join our Emergency Department and Inpatient Behavioral Health teams. The role conducts psychosocial assessments for adults, adolescents, and children to identify barriers to recovery and healthy living, and provides consultation to the clinical team.

Qualifications

• MSW degree required.
• CT licensure in appropriate professional designation required (e.g., Clinical Social Worker, Psychologist, LMFT, or LPC).
• Excellent written and verbal communication; knowledge of community resources.

Responsibilities

• Performs psychosocial assessments for adults, adolescents and children in ED and inpatient behavioral health.
• Provides support to patients and families; acts as patient advocate to ensure appropriate care.
• Evaluates patient safety and makes referrals for treatment in coordination with the healthcare team.
• Communicates with patients, families and the clinical team to ensure a safe discharge plan and continuity of care.
