Job summary

The Health Center at Bloomingdale is seeking a Certified Occupational Therapist Assistant (COTA/ OTA) on a per diem basis to support the rehabilitation plan of each resident. The assistant collaborates with the interdisciplinary team and supervises aides under state guidelines.

Responsibilities include providing rehab care, documenting progress, communicating resident responses to therapy, and assisting the supervising therapist to ensure quality outcomes in daily center operations.

Qualifications

• Graduate with an associate's degree in physical or occupational therapy.
• Licensed/certified as COTA/OTA in the designated state.
• Active licensure/certification in the employed state.

Responsibilities

• Provides resident rehabilitation care under the plan of care.
• Communicates resident condition with supervising therapist.
• Completes required therapy documentation as directed.
• Provides feedback to therapists and Rehabilitation Director.
• Participates in center processes and in-service training.
• Manages treatment and non-treatment activities per plan.
