Job summary

Vanderheyden Hall Inc. is seeking a Community Habilitation Specialist to coordinate and assist individuals and families in achieving specific goals through respite and residential habilitation services, skill building, and parent assistance. The role is a Per Diem position paying $20/hr.

duties include direct care, goal-oriented activities, and documentation. Responsibilities emphasize scheduling, reports, and collaboration with staff and community resources.

Qualifications

• Valid NYS Driver’s License with clean driving record.
• High School diploma or GED.
• Ability to manage time and workload effectively.
• Problem solving skills.
• Ability to communicate effectively, orally and in writing.
• An equivalent combination of education, experience, knowledge, skills, abilities.

Responsibilities

• Maintain knowledge of community-based resources including, but not limited to, age-appropriate recreational activities.
• Provide direct care and services to individuals and families.
• Assist individuals with goal orientated activities based on agreed upon services plan.
• Attend and participate in supervision, staff development opportunities and agency/community committees.
• Communicate and interact with individuals and families appropriately and effectively.
• Explain and promote Sanctuary to prospective employees, volunteers and the community.
• Create personal schedule based on individuals served utilizing appropriate time management techniques.
• Complete written service summaries as required.
• Complete assigned paperwork, including intake assessments and discharge summaries in a timely manner.
