Job Summary

The Clinical Triage Specialist (CTS) (RN) in the Access Center provides compassionate patient support, utilizing nursing judgment to triage calls and offer clinical guidance based on protocols. The role involves assessing symptoms, prioritizing care, and coordinating services to ensure optimal patient outcomes.

Responsibilities

1. Answer and prioritize triage calls, follow protocols, and coordinate services.
2. Verify and update patient demographic information in the electronic health record.
3. Serve as an escalation point for clinical issues and provide advice based on protocols.
4. Respond to electronic patient messages, prioritizing and addressing concerns.
5. Create a positive patient experience, resolving issues within scope.
6. Meet productivity, schedule adherence, and quality standards.
7. Assess, advise, schedule, or refer calls to appropriate personnel or facilities.
8. Document symptoms, assessments, advice, and patient responses accurately.
9. Collaborate with clinical teams and practices on patient concerns and scheduling.
10. Perform other duties as assigned.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in Pennsylvania or seeking licensure via reciprocity.

Experience and Skills

• Minimum 2 years recent clinical experience in physician's office, home health, critical care, or emergency room.
• Strong communication, problem-solving, and time management skills.
• Focus on compliance, quality, and service orientation.
