Qualifications

• Bachelor's degree in Pharmacy.
• Pharmacist License.
• One year of related experience.
• Doctor of Pharmacy (PharmD) preferred.
• Pharmacy Residency training preferred.

Responsibilities

• Directs clinical activities of the pharmacy service area.
• Reviews physician orders for indication, dose, allergies, interactions.
• Contacts physicians when questions arise; document interventions.
• Ensures medications are prepared, labeled correctly and compliant.
• Provides drug information using literature and references.
• Participates in medication education and reconciliation processes.
• Engages in quality programs and population health initiatives.
