Job summary

VytlOne is seeking a per diem Staff Pharmacist to join the Hudson Headwaters Health Network in Malone, NY. You may cover the Pharmacist in Charge on absence, leave, or other times as needed and provide high-quality patient care.

The role requires knowledge of drugs, patient counseling, and collaboration with physicians; vaccines are required, and clinic-based work supports a team-focused environment. Experience as a practicing pharmacist is preferred.

Qualifications

• Knowledge of drugs, indications, dosing, and safety.
• Ability to communicate technical information to others.
• Familiarity with policies governing pharmaceutical disbursement.

Responsibilities

• Interpret and fill drug orders.
• Provide patient information and counseling.
• Act as alternate lead Pharmacist-In-Charge when needed.
• Contact physicians regarding prescription questions.
• Dispense prescriptions and prepare I.V. admixtures if applicable.
• Monitor therapy for interactions, allergies and appropriateness.
• Maintain medication records and inventory.
• Supervise staff and participate in training.
• Maintain compliance with regulatory and ethical policies.
