About The Role: As a Certified Home Health Aide (CHHA), you will provide essential care and support to patients in their homes, promoting independence, comfort, and dignity, while ensuring the highest standard of care.

Responsibilities Include:

• Measuring and documenting vital signs during visits.
• Assisting with personal hygiene, bathing, grooming, and oral care.
• Supporting mobility, transfers, ambulation, and prescribed exercises.
• Preparing nutritious meals, including therapeutic diets, and assisting with feeding.
• Helping with medication reminders (not medication administration).
• Performing light housekeeping tasks like changing linens and cleaning.
• Observing and reporting changes in patient condition to the supervising nurse or therapist.
• Following care plans and assisting with specific needs under professional guidance.
