Job summary

Select Specialty Hospital in Omaha, NE is seeking a Case Manager (PRN) to lead discharge planning, utilization reviews, and social work services. You will coordinate with payors, manage treatment plans, and ensure compliant documentation under the Direction of Case Management.

Qualified candidates must hold current licensure (RN/LPN/LV/N, RT, or SW/MSW) and have inpatient critical care experience. This is a part-time, as-needed role with a $42/hr pay rate and a $2,500 sign-on bonus.

Qualifications

• Current licensure in a clinical discipline (RN/LPN/LVN or RT) or SW/MSW per state guidelines.
• Previous inpatient hospital experience with critical care/acute care patients (ICU, step-down, med surg, vents).
• Experience in acute medical case management and ability to direct a plan of care for chronically critically ill populations.

Responsibilities

• Develops patient-specific, safe and timely discharge plans.
• Performs verification of utilization criteria reviews.
• Coordinates with payor sources to ensure proper reimbursement for hospital services.
• Maintains compliance with Utilization Management policies and procedures.
• Facilitates multidisciplinary team meetings with physicians, nurses, and therapists.
