Job summary

MemorialCare is seeking a Transitions of Care Pharmacist on a per diem basis in Long Beach, CA. The role focuses on accurate medication reconciliation, evidence‑based management, and safe transitions between inpatient and outpatient settings.

Under supervision, you will collaborate with physicians, nurses, case management, and outpatient pharmacy teams to reduce medication‑related harm and improve TOC outcomes. Requires CA licensure and 3+ years in acute care.

Qualifications

• Active CA registered pharmacist license required.
• Minimum of 3 years of clinical pharmacy experience in an acute care setting.
• Direct experience performing admission and/or discharge medication reconciliation preferred.
• Completion of an accredited PGY1 Pharmacy Residency or PGY2 Transitions of Care Pharmacy Residency strongly preferred.
• Strong communication and collaboration skills with a multidisciplinary team.

Responsibilities

• Obtain the best possible medication history for high‑risk patients on admission and perform admission reconciliation.
• Discharge medication reconciliation for high‑risk patients with accurate, appropriate lists.
• Support quality improvement initiatives to improve reconciliation accuracy and workflow efficiency.
• Contribute to development of TOC workflows, guidelines, and training for staff.
• Interact professionally and be punctual, following company policies.
