Job summary

Advocate Health in Wisconsin seeks a licensed physical therapist for outpatient orthopedic and pelvic health. The caseload splits between pelvic floor and orthopedic, at 9200 W Loomis Rd, Franklin. This is a full-time, 40-hour-per-week position with two late shifts; benefits eligible and a $5,000 sign-on bonus.

Responsibilities include evaluating referrals, planning care, monitoring outcomes, documenting care, and collaborating with the care team to optimize patient outcomes.

Qualifications

• Doctorate degree in Physical Therapy from an accredited program.
• Current state licensure in the state of practice (WI).
• Maintains continuing education requirements as mandated.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation results and goals.
• Monitors outcomes and adjusts treatment plans; communicates progress to physicians and teammates.
• Documents patient evaluations, progress notes, discharge summaries, and other required records.
• Maintains productivity standards and ensures safe, compliant care delivery.
• Orients and mentors staff as needed and may supervise PT assistants or extenders.
