Job summary

Louisville Metro Government seeks a dedicated social service worker to implement and run peer mentoring and support programs for homeless individuals and those in permanent supportive housing. You will help clients develop recovery plans, provide illness-management education, and coordinate services as an extension of case management.

You will also train peer mentors, support street outreach, and conduct home visits to reinforce life skills and recovery while assessing client needs and matching

Qualifications

• High school diploma or equivalent.
• Must complete Kentucky Peer Support Specialist training and certification within one year of employment and maintain certification.
• Equivalent combination of training and experience may be substituted by Civil Service.

CHARACTERISTICS OF THE WORK

Under direction, performs social service work.

Responsible for implementing and conducting peer mentoring and support programs for homeless individuals and/or individuals receiving permanent supportive housing services.

• Provides support and encouragement to help clients strengthen life skills and recovery.
• Assists in training peer mentors on providing peer support services.
• Accompanies street outreach team to provide peer support for clients served at street outreach sites.
• Conducts home visits to clients receiving permanent supportive housing to provide life skills training and peer support, and to complete assessments of client needs.
• Assists in the process of determining appropriate matches between peer mentors and recipients of mentoring services.
• Performs related work as required.

MINIMUM REQUIREMENTS

• High school diploma or its equivalent.
• Must have successfully completed the Kentucky Peer Support Specialist training and certification within one year of employment and maintain certification throughout employment with FHC.
• An equivalent combination of training and experience may be substituted, as determined applicable by Civil Service.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

Spends prolonged periods of time standing and walking over various types of inner-city terrain.

SPECIAL REQUIREMENTS

Must provide own transportation to work at other locations as needed.
