Job summary

IMind Health is hiring a Community Support Worker to provide direct support and case management to participants living with severe mental illness in the Lanham area. This community-based role involves home visits when appropriate, building rapport, and coordinating services to promote stability and community integration.

The position emphasizes engagement, documentation in EHRs, and collaboration with a multidisciplinary team.

Qualifications

• Experience providing case management or behavioral health services to individuals with severe and chronic mental illness.
• Experience with PRP and/or MTS or similar services is preferred.
• Strong interpersonal, verbal, and written communication skills.
• Ability to work in community-based settings and in participants’ homes when appropriate.
• Proficiency with basic Microsoft Office and EHR documentation.

Responsibilities

• Provide direct support, case management, and rehabilitation services to program participants.
• Develop and maintain professional relationships with participants and build trust.
• Assist participants with identifying needs, setting goals, and connecting to resources.
• Accompany or transport participants to appointments and community activities.
• Assist participants with accessing social services, housing resources, healthcare, transportation, and other supports.
• Provide services in community settings and participants’ homes when safe and appropriate.
• Support participants in developing skills for independence and stable living.
• Recognize and de-escalate behavioral health concerns as needed.
• Complete timely documentation in EHRs and other systems.
• Maintain confidentiality and comply with program requirements.
• Participate in team meetings and care planning.
