We’re hiring for a Peer Support Specialist to join our Outreach and Engagement team in the SOS program.

Hours: Monday through Friday 8:00 am to 5:00 pm

What You’d Be Doing

As an outreach Peer Support Specialist with Outreach and Engagement in the SOS program, you would play a central role in helping individuals in the community that would otherwise be engaging with law enforcement.

Specifically, the Peer Support Specialist:

  • Supports the new client process through completion of necessary paperwork and screenings for enrollment into services;
  • Makes appropriate referrals to address mental health and substance use needs;
  • Conducts client home visits;
  • Provides or connects to supportive housing services.
  • Collaborates with other treatment providers to ensure a team based approach to treatment, when applicable
  • Identifies coping mechanisms and natural supports that may be available;
  • Teaches and enhances client coping, symptom management, problem-solving, independent living, and socialization skills;
  • Maintains competency and proficiency with our electronic health record system;
  • Provides support to remain in home and community to avoid more restrictive living arrangement; and
  • Performs other job-related duties assigned by the director or his/her designee.
  • Physical requirements include the following:
  • Must be ableto remain in a stationary position at least 50% of the time.
  • Frequently operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
  • Occasionally lifts and carries items up to 15 pounds.
  • Occasional exposure to communicable diseases and bodily fluids.
  • Occasionally drives, at times in adverse weather conditions.

Qualifications

What We’re Looking For—The Must-Haves

  • High School Diploma or GED
  • Personal knowledge or experience in the behavioral health field/housing
  • Computer proficiency
  • Colorado driver’s license, a good driving record, and access to transportation during work hours

  • Experience in the area of supportive housing services
  • Experience successfully working in multi-disciplinary teams
  • Experience working with adults with severe and persistent mental illness
  • Experience providing case management services to adults
  • Community systems navigation background
  • Spanish language skills
  • CAC training
  • Experience working with those experiencing co-occurring disorders
