Job summary

Abode Services in San Jose, CA is seeking a Peer Specialist for Blossom Valley Senior Apartments program to provide outreach, case management, and rehabilitative services to participants identified as high users of emergency services.

The role emphasizes engaging individuals with Serious Mental Health Disorders, Substance Use Disorders, or chronic health conditions, connecting them to mental health, health care, housing, and benefits, and supporting self-sufficiency goals.

Qualifications

• High school diploma or equivalent.
• Personal life experiences with recovery from addiction, homelessness, and/or mental health recovery.
• Six months of field experience working with people experiencing homelessness.
• Valid California Driver’s License, current insurance, and clean DMV record.

Responsibilities

• Provide outreach and engagement to homeless adults to locate referred individuals.
• Re-engage participants who have lost contact with the team.
• Assist in interviewing and completing initial intakes, gathering information, and maintaining records; keep thorough case notes.
• Link participants with community treatment services for mental health, substance recovery, physical health care, financial assistance, housing and benefits.
• Help participants establish goals and strategies for increasing self-sufficiency and accessing public benefits.
• Support participants in navigating the criminal justice and court systems and advocate as needed.
• Develop Wellness Recovery Action Plans and provide coaching to participants.
• Attend clinical, administrative, and case conferencing meetings as dictated by the supervisor.
• Enter data timely to support program evaluation and billing (Medi-Cal).
• Assist Program Manager and staff as needed.
