Peer Specialist - Assertive Community Treatment

A community-based mental health program is seeking a Certified Peer Specialist to join its Young Adult ACT (Assertive Community Treatment) team. This hybrid field-based position provides peer-driven support and recovery-oriented services to individuals aged 18-28 living with serious mental illness.

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 9am to 5pm
  • Location: Office in West Babylon, NY | Field-based services with some remote flexibility

Responsibilities:

  • Provide strength-based peer counseling and recovery coaching
  • Conduct outreach visits and support in community-based settings
  • Assist with transportation to appointments and facilitate community integration
  • Support clients during transitions, including hospital discharges and crisis situations
  • Maintain detailed and timely documentation in line with program requirements
  • Submit progress notes tied to treatment plans within 48 hours
  • Assist with intake documentation, assessments, and service plans as assigned
  • Document follow-up after client discharge at 30, 60, and 90 days
  • Facilitate or co-facilitate peer support groups as appropriate (not currently required but may be added)
  • Ensure all documentation, scheduling, and expense reporting (via Concur) is completed on time
  • Perform other duties as needed

Qualifications:

  • High School Diploma / GED
  • Current certification as a NYS Peer Specialist and/or Youth Peer Advocate
  • Lived experience relevant to mental health and/or recovery (essential and evaluated in the interview)

Desired Skills:

At least 1 year of experience in a Treatment, Rehabilitation, or Peer Support role
