This role provides hope, advocacy, mentorship, and guidance to SAGE clients navigating both the justice system and treatment. The PRSS works one-on-one and in group settings to foster engagement, promote recovery, and connect clients with essential community and treatment resources. This position holds a caseload of approximately 50 clients, primarily referred to by justice-involved pathways, including outpatient and transition programs.

Key Duties:

  • Assess and address social determinants of health that impact progress toward goals, including intensive facilitation of access to resources and care coordination (including, but not limited to, facilitating connection to primary care, recovery groups, behavioral health homes, medication assisted treatment, and employment).
  • Utilize coaching, mentoring, and role modeling using lived experience according to established PRSS credentialed practices. Interventions may include, but are not limited to, identifying needs and recovery goals, lessening feelings of isolation, increasing practical knowledge to access supports and resources in the community, exploring continued education and/or employment opportunities, partnering with other healthcare practitioners, and overcoming service barriers.
