Theuniversityunion is seeking a Part-Time, non-exempt Peer Linkage Coordinator in Solon, Ohio. You will connect peers to recovery services and coordinate care with jail staff for successful re-entry, while maintaining confidential records and engaging stakeholders.

As a certified peer supporter with recovery experience, you will advocate for peers and help them access resources to improve health and wellness outcomes.

Qualifications

  • Identify as a person in recovery with at least one year of successful recovery.
  • Certified Peer Supporter in the state of Ohio.
  • Knowledge of Buckeye program and linkage processes.
  • Working knowledge of community resources and transportation options.
  • Proficient with Microsoft Office and record-keeping.

Responsibilities

  • Connect peers to ongoing peer support services and link to community resources.
  • Conduct outreach to peers to engage them in resources prior to release.
  • Coordinate re-entry planning with onsite jail staff for release.
  • Provide information to help individuals access services for health goals.
  • Maintain accurate records, notes, confidentiality, and timely reports.
  • Enter notes accurately in files and computer databases.
  • Advocate for the needs of each individual and complete caseload documentation.
  • Collaborate with internal/external teams to manage workflows and care coordination.
