Theuniversityunion is seeking a Part-Time, non-exempt Peer Linkage Coordinator in Solon, Ohio. You will connect peers to recovery services and coordinate care with jail staff for successful re-entry, while maintaining confidential records and engaging stakeholders.
As a certified peer supporter with recovery experience, you will advocate for peers and help them access resources to improve health and wellness outcomes.
Responsible for connecting peers to ongoing peer support services and linking them to critical resources in their communities
Conduct in reach to peers to engage them in resources and services prior to release
Coordinate re-entry planning with onsite jail staff in preparation for release
Provide information and opportunities for individuals to begin to find independence in accessing services to better their health and wellness goals
Responsible for maintaining accurate records and notes, confidentiality, and compiling timely reports
Through identifying as a person in recovery and intentionally sharing their recovery story; use this as appropriate means to assist clients, provide hope, and help in changing patterns and behaviors
Help peers understand recovery, as well as identify and achieve their own recovery needs, wants and goals
Maintain records and provide assistance with appropriate documentation in files and computer database
Achieve all performance goals in regard to client outreach, engagement and services provided
Enter all notes in an accurate and timely manner including phone notes
Advocate for the needs of each individual
Complete caseload management including all necessary documentation
Communicate with stakeholders to build a rapport and coordinate care
Develop working relationships with external contacts and collaborate across teams
Collaborate with internal and external teammates to manage workflows
Assist teammates in learning process related programming
Manage relationships with external treatment agencies and other providers
Other duties as assigned
Identify as a person in recovery with at least one year of successful recovery
Certified Peer Supporter in the state of Ohio
Knowledge of Buckeye program and linkage processes
Working knowledge of available resources within each community
Understands problems analysis, decision-analysis, evaluate interventions, and contingency plans
Ability to be an active listener by asking questions and reflecting on what is being said
Knowledge of area resources that may aid client in ongoing recovery ( i.e. transportation, treatment providers, sober support systems)
Proficient level of ability with computer; Microsoft Office Products
Effective written and oral communication skills; ability to articulate information clearly
