Qualifications

Responsibilities

  • Build rapport with members and encourage positive behavior changes.
  • Coach members to create strategies reducing health risks.
  • Connect members to programs to address barriers to care.
  • Link members to local, county, and state services.
  • Deliver medications and supplies as needed.
  • Escort members to medical and social services appointments.
  • Introduce self-management tools to promote self-efficacy.
  • Complete documentation in EHR and care plans.
  • Update HMIS/Coordinated Entry System records.
  • Schedule encounters based on member acuity.
  • Work under supervision of a Social Worker for escalation.
  • Occasionally drive the medical van.
  • Support housing placement goals for unsheltered members.
  • Perform other duties as assigned.

Peer Navigator – Healthcare in Action

Position: Full‑time, Monday‑Friday, 8‑5 schedule.

Healthcare in Action serves patients experiencing homelessness through an innovative “street medicine” approach, delivering medical care directly with a medically equipped van to locations such as parking lots, encampments, and parks. This role supports members in Riverside, CA, helping them manage their health and achieve housing placement. We seek rebels who are curious about AI and its power to transform how we operate and serve our members.

Responsibilities
  • Build rapport with members by sharing personal experiences to encourage positive behavior changes.
  • Coach members to create and implement strategies that minimize risks associated with common health conditions and behaviors.
  • Connect members to appropriate programs to address barriers to care and enhance compliance.
  • Link members to local, county, and state services/resources.
  • Follow up with members and serve as an advocate, delivering medications and supplies as needed.
  • Coordinate and/or accompany members to medical and social services appointments outside of the street medicine unit.
  • Introduce tools to promote self‑management and self‑efficacy.
  • Complete all required documentation in a timely fashion, including recording patient touch points, encounter notes, risk assessments, and updating care plans in HIA’s electronic health records system.
  • Complete a Monthly Audit Checklist assessment, individual care plans, and update member profiles in the Coordinated Entry System/HMIS.
  • Schedule encounters based on member acuity using HIA’s electronic health record system.
  • Work under the supervision of a Social Worker to ensure proper escalation of cases and coordination of care.
  • May occasionally drive the medically equipped van.
  • Actively support the achievement of HIA’s vision and goals, particularly housing placement for unsheltered members.
  • Perform other duties as assigned.
Qualifications
  • GED/High School diploma required, plus 3+ years of related work experience. Associate’s degree preferred, with 2+ years of experience; Bachelor’s degree preferred, with 1‑2 years of experience.
  • Direct work experience, lived experience, training, or volunteer work with patients experiencing homelessness, including those with active mental health and substance use conditions.
  • Relevant experience in similar community settings, such as working with disadvantaged groups or outreach programs.
  • Ability to work in a dynamic, outdoor environment.
  • Ability to work independently and as part of a team.
  • Strong organizational and prioritization skills.
  • Excellent oral communication and interpersonal skills, with the ability to articulate personal experiences.
  • Reliable transportation and a valid driver’s license, automobile insurance, and clean driving record. Must be able to travel 50% or more of the time.
  • Proficiency in operating computers, including time‑keeping and call center software; experience with HMIS/Coordinated Entry System preferred.
