Full-time and part-time positions are available. Case-specific Enhanced Pay Rates are offered for 8-12 shifts. We provide flexible hours, weekly pay, and a supportive work environment.

Responsibilities

Job Summary

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by physicians or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care by sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Deliver high-quality clinical services within scope of practice and infection control standards, according to the care plan, coordinating with the care team from admission to discharge.
• Perform clinical nursing assessments in line with federal and state requirements and payer specifications.
• Verify patient eligibility and medical necessity for services based on payer source and agency policies.
• Develop and update individualized care and service plans in collaboration with community providers, adhering to federal and state regulations.
• Adjust care plans as needed, with physician approval, to meet patient needs.

Education and Experience

License Requirements

• Current RN licensure in the practicing state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.

State-Specific Requirements

• LA: At least one year of clinical RN experience; this can be waived for RNs with recent LPN clinical experience. RN licensure must be unrestricted.
