Job summary

UnitedHealth Group’s United States home health team in Louisiana is seeking a Licensed Practical Nurse to provide direct patient care in the home setting and to document patient visits per regulations. You will work under supervision and coordinate with the care team to ensure quality care for acutely and chronically ill patients.

Qualifications

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state law and plan of care.
• Documents patient visits thoroughly in the electronic medical record and VE program as applicable.
• Participates in care coordination, communicates patient needs and changes in condition to the team.
• Ability to work in home settings with diverse patients and communities.
