Responsibilities

The Licensed Practical Nurse (LPN) in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, following physician prescriptions and applicable laws, regulations, and agency policies. The RN collaborates on care plans and ensures continuity of care by communicating with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Provide high-quality clinical services within your scope of practice and infection control standards, coordinating with the care team from admission to discharge.

Complete clinical nursing assessments as required by federal and state programs and payers.

Verify patient eligibility and medical necessity for services.

Develop and update individualized care plans in accordance with federal and state requirements, collaborating with community providers.

Adjust care plans as needed with physician approval to meet patient needs.

Experience and License Requirements

• Minimum one year of RN experience; PEDS experience preferred
• Current RN licensure in the practicing state
• Valid driver’s license, vehicle insurance, and access to reliable transportation
• Current CPR certification
